About supported living

Our aim is to help customers to live independently in their own home. We work with partners within social services, support and care teams, and mental health teams. Together we can support customers with:

  • developing independent living skills
  • managing rent and bills
  • applying for benefits
  • furnishing homes
  • managing everyday tasks and care needs
  • exploring and learning about the local area
  • installation of specialist equipment
  • installation of wheelchair access

Supporting customers to live in their home

Our Partnerships Co-ordinator visit customers on a regular basis to:

  • Make sure they’re getting the benefits they’re entitled to
  • Carry out health and safety checks in their home
  • Create and regularly review their support plan and budget plan
  • Make sure repairs are reported and completed
  • Support them to stay safe, secure and heard
  • Support them to report safeguarding concerns
  • Help to understand the rules of the tenancy agreement and how to maintain positive relationships with other customers living within the scheme
  • Actively listen to feedback about the service or scheme
  • Ensure choice by involving the customer, their family or advocate in decisions that affect them and their home.
