Form to be complete once the work has been done.
Please submit this form once you have completed the improvements or adaptations you requested permissions for.
Once you submit the form, we will contact you via email within five days to either confirm and sign off the work, or arrange a visit if a further inspection is required.
Be as detailed as possible and upload any images and relevant certificates. The more information given may mean we do not need to visit to sign off the work.
